What is this?
A follow-up program specifically aimed at those of you who have been diagnosed with atrial fibrillation and want close and personal follow-up over time.
Why this program?
Atrial fibrillation is a complex condition – not just a simple diagnosis. Symptoms, triggers, and treatment needs vary greatly from person to person. What works for one patient may not be right for another.
Treatment should therefore be adapted individually, with regard to:

Other medical conditions
However, there are currently limited opportunities for tailored follow-up in practice – especially after diagnosis. This program was developed precisely to explore how close, personal and flexible follow-up can provide better control, fewer symptoms and more security in everyday life.
A low-threshold pilot project
The program is offered as part of a pilot project, and therefore at a reduced price. The goal is to test a new way to follow up patients with atrial fibrillation – with weekly video consultations over 3 months.
